Making a referral
It is not possible for clients to self refer, however we welcome enquiries, please use the link to the right. All referrals must come from a health care provider/professional.
Our referral form for use by health care providers/professionals is available to download (Adobe Reader required) or by contacting the unit directly by telephone 01273 645 694.
Health care providers/professionals
Please note we now have a new referral form that you can download, type in and e-mail direct.
If you use Outlook/Outlook Express
Simply fill in all relevant fields in the form and push the Submit Form button at the end of Page 2.
If you use a web based e-mail service such as Hotmail, Googlemail etc.
Simply fill in all relevant field in the form and save the completed PDF to your computer. Log into your e-mail and attach the PDF file and e-mail to referrals at sussexbeacon.org.uk
