We recognise that confidentiality is an important part of a person’s care. All patients and clients should be able to expect that any information given to any member of staff within The Sussex Beacon will be held in a secure and confidential manner.

We need to know information about you so that we can provide the best care and support for you and refer you to other services if needed.

What information do we collect?

To enable us to provide services for you and meet your needs we have to keep records by law. Our patient records are held in paper form and electronically. Your record may include information such as:

• Name, address, NHS number and date of birth
• Your medical history and details of any allergies
• Assessment, care plan and reviews
• Details of other people who are involved in your care. For example, your GP

It is important that you tell us on any changes to your personal details.

How do we keep your information safe?

All our staff are trained in confidentiality and how to keep your information safe.
We keep paper records safely locked away, use passwords for computers and mobile phones, only share information with people who are involved in your care and staff make sure that they know who people are if they ask for information about you.

Who do we share information with?

We will only share information with people who are involved in your care. This could be your doctor, nurse, physiotherapist, occupational therapist, GP, social services or another organisation who are helping you, such as a charity.

Caldicott Guardian

All health and social care organisations are required to have a ‘Caldicott Guardian’. The Guardian is a senior person with responsibility for ensuring that we meet the highest standards for handling patient identifiable information and maintaining confidentiality.

Our Caldicott Guardian is: Jason Warriner, Clinical Services Director.

If you have any questions about confidentiality, please speak to the member of staff who is looking after you or ask to speak to the manager of that service.
